Instructions for Customizing Your Volunteer Registration Form

The PIT Volunteer Registration Form can be used to assist the Simtech team in modifying the Volunteer Registration Portal to meet your local needs.  In addition to collecting key information and automatic account creation for volunteers, the available volunteer registration feature can be used to manage volunteer distribution within the Command Center using the region management functionality. Both the Volunteer Portal and the Region Management functionality are options available to communities who elect to add additional features.

 

What we need to create your volunteer portal:

  1. General information about your count for your landing page
  2. Your CoC or PIT Count logo (optional)
  3. Questions you wish to include within your form (Default questions, COVID-19 questions, Custom questions)
  4. Volunteer release form language
  5. E-mail confirmation message

 

Please complete this survey to submit the required information and select the questions you wish to include in your region’s Volunteer Registration Form. Once the survey has been completed, the Simtech Team will update the survey and share for your review and approval.
